Kodokan Judo Association (SA)

I hereby apply for membership/renewal/upgrade of membership of the Sakura Kodokan Judo Association Inc. trading as Kodokan Judo Asscoiation (SA)-KJA(SA).

I/my child agree to abide by the constitution of the KJA(SA) and /or any rules,by-laws or policies created or enacted by the Association.

In so applying and in consideration of my/mychild's application being accepted, I have read, acknowledged and agreed to the declaration included in the warning, exclusion of liability and indemnity clauses.

I acknowledge the the KJA(SA) may reject this appilcation at its discretion. I acknowledge that if my/my child's application for membership is successful I/my child will be entitled to all benefits, advantages and services of membership.
